T-Mobile Phone Service and App Options for MAPs Graduates
1. T-Mobile Phone Service Standard Termination (Default)
If you have been provided a phone through the MAPs project, your T-Mobile phone service will terminate on your MAPs program end date. This service has been provided to you by AbleLink under contract with TN DDA. All personal data and information on your MAPs phone will be erased. This will remove all personal information on your phone – including, but not limited to, all installed apps, downloaded videos and music files, texting history, pictures, phone call history, and contacts. You will need to back up any information you wish to save prior to your MAPs program end date. If you have questions, please contact AbleLink.
Ownership of the phone device and any protective case provided to you through the MAPs program is automatically transferred to you upon your MAPs program end date. All technical support for your phone will also end on this date. You will be able to set up new phone service with any mobile service provider for use with your phone. If you wish to keep your existing phone number and transfer it to another mobile service provider, contact AbleLink MAPs support.
2. Continue T-Mobile Phone Service and App Access with AbleLink
You may choose a private pay option to continue T-Mobile service on your phone and keep your existing apps and personal data without resetting your phone. If you choose this option, your phone will not be reset at the end of your MAPs program participation. Your existing phone service from T-Mobile will continue uninterrupted, along with support for the AbleLink Apps on your phone. Your device will also continue to be managed by AbleLink through the Mobile Device Management (MDM) system currently in place.
To request the private pay option, contact AbleLink at least 30 days prior to your MAPs program end date. If you do not specifically select this service option, your T-Mobile service will be suspended on your MAPs program end date (see description above).
Standard Rate: $200 per month (funding may be available for individuals eligible for a DDA Medicaid Waiver)
Discounted Rate: $100 per month (a discounted rate may be available through the Davies-Gauthier Foundation for previous MAPs participants who are not eligible for Medicaid waiver funding)
Note: This option requires agreement to AbleLink’s Enabling Technology / T-Mobile Service contract and provision of a credit card for recurring monthly payments (or service authorization through a TN Medicaid waiver). This service can be terminated at any time with 30 days’ written notice by contacting AbleLink.
3. Graduates Using Personal Phones During the MAPs Project
Software licenses for the AbleLink Apps (Endeavor, Visual Impact, and WayFinder) will be discontinued upon graduation from the MAPs program. Content on these apps will no longer be editable and there will be no further support available from TN DDA or AbleLink. These apps can be uninstalled at any time after completion of the MAPs program. If you wish to continue using the AbleLink Apps on your personal phone, contact AbleLink for information on the Enabling Technology Service.
